New Patient Forms

Prior to your initial visit, there is some much loved paperwork that we require you to fill-out. The file can be found below, or can be picked up at the Nature Med office. Depending on the complexity of your medical issues it may be beneficial for Dr. Kara to review the forms prior to your visit. If you feel this is the case, you may fax, email or drop off the forms at the office prior to the visit date.

Copies of your most recent laboratory results (e.g. blood work) and/or medical reports (e.g. x-rays, ultrasound etc).

*Note: Generally the easiest way to obtain these reports is to ask the receptionist at your doctors office to copy them for you (rarely, a fee may be charged).
*Did you know? By law, you have the right to access your medical records. It is always advised to keep your own copies...be proactive in your health care!

Bring a detailed list or the actual bottles of your current medications and supplements you are taking or have taken in the past (including dosages and frequency)
